Details

Firm license fee$10
$10 per year per manufacturing/distribution facility; applicant may choose a 1-year or 2-year license ($20) at application
Exempt
  • Treats making no nutritional/health claims and not claiming to be complete and balanced: 'In Arizona if you stick to making no nutritional claims, no health benefits and make no claims to be complete and balanced for any or all life stages, a feed license is not required.' (Animals FAQ)
Tonnage noteNo small-package exemption. $0.20/ton rounded to whole ton, $2.00 minimum per quarter even with zero distribution ('A report MUST be filed and $2.00 minimum paid even if tonnage is zero'); quarterly statement: 'DUE DATE is the 15th Day of Succeeding Quarter. LATE FEE applies if postmarked more than 30 days past the quarter ending date' — late fee $10 or 10% of fee due, whichever greater. After one year, firms under 400 tons may elect 'estimator' status: one annual payment ($8 per FAQ) using the Annual Sales Estimate form plus a year-end verification form.
Third-party filingagent_signature: “License application: 'Applicant Name/Title … The undersigned hereby makes application for a new commercial feed license, pursuant to A.R.S. 3-2609' plus an optional 'If tonnage fees are not paid from Licensee's mailing address, please provide proper address below: Division or c/o: / Tonnage Address' block. Quarterly tonnage statement: 'PRINT or type name of authorized person / Title / Signature'; AAC R3-3-902(B) requires 'the name, title, telephone number, and signature of the licensee or the licensee's authorized representative' on tonnage reports. So a c/o address and an authorized representative are accepted on paper; no agent login role is published (feed packet PDF, re-read 2026-10-08).”
Before you file
  • Federal Tax ID required ('Application will not be processed without a tax ID number'); FAQ says sole proprietors may use SSN
  • Copy of each feed label intended for AZ distribution (proof sheet or electronic copy acceptable; no packaging)
  • Separate application per manufacturing or distribution facility

How the online filing goes

  1. Online or paper?
  2. New application — Agriculture Applications (from the 'Apply or Renew' instructions)
  3. Application details (expect the same questions as the paper form; labels not seen on-screen)
  4. Renewal — Credentials Online (renewals.azda.gov/credentials)

Your packet lists every value for each screen. Online: eCheck/ACH (no fee) or Visa/MasterCard/AmEx/Discover ($2.00 or 2.2% processing fee, whichever is greater, since Aug 22, 2025). Paper: check payable to 'Arizona Department of Agriculture ESD' mailed with the application to Central Licensing Section, 1802 W Jackson St., #78, Phoenix, AZ 85007. In person at 1010 W Washington St., Phoenix: card, cash or check. The license fee is $10 per year ($20 for the 2-year option).

Forms

Good to know

  • Cheapest firm license in the set ($10/yr) but tonnage never goes away: $2/quarter minimum or $8/yr estimator — a $0 report still owes $2.
  • Odd fiscal cycle: July 1 – June 30, with a 2-year option.
  • No-claims treats may be exempt from licensing entirely (Animals FAQ) — label still needs product name, ingredients, net weight, manufacturer name/address.
  • Tonnage report forms are mailed to the address of record each quarter; non-receipt is not an excuse.
