$80 per product per calendar year, any package size ('Calculated @$80.00 per product'); non-refundable; each new or renewed registration

Forms

Good to know

  • No firm fee, no tonnage — cost is purely $80 × products × year; one of the pricier per-SKU states for a multi-flavor treat line.
  • Renewal form is deliberately not online; must phone/email the licensing unit each cycle.
  • Form is built for agents: separate Registrant/Agent column vs Company column and an 'on behalf of' certification.
  • Dog biscuits are pet food and 'each feed product formulation must be registered' (Agricultural Commodities page).
  • Posted application still shows 'Registration Expiration: 12/31/23' and 'feed-app-2024.pdf' variant exists — the form is reused year to year.
  • Failure to register a feed after notice: $100 administrative fine per product (§22-118l(d)(2)).
