Forms

Good to know

  • License form has a separate 'Tonnage Mailing Address' block and feed-category checkboxes ('Small Package (10 lbs. or less)' / 'Bulk/Over 10 lbs.').
  • Specialty pet food has its own form and a $15 bracket at ≤1 lb; dog/cat food uses the $25 ≤10 lb form.
  • agriculture.ks.gov is behind Akamai and returns 403 to non-browser fetches; PDFs here were pulled from the Wayback Machine (2024 snapshots) – verify revision dates before filing.
