Forms
Good to know
- The '$100 per pet' on the form means $100 per pet-food product name ($80 fee + $20 spay/neuter surcharge) – highest flat per-product fee in this batch, no small-package discount.
- Home-based manufacturers (≤10 product names, made at home, sold direct to consumers) pay one $80 + $20 total regardless of product count.
- Surcharge money is earmarked: first $100k/yr to Companion Animal Sterilization Fund, then Animal Welfare Fund.
- Multi-year (2- or 3-year) registration can be requested to skip annual paperwork.
- Same form covers feed, seed, fertilizer, lime, soil amendments – tick 'Pet Food' and the per-product fee line.
