Forms

Good to know

  • Home-kitchen treats allowed only if shelf-stable, non-perishable (biscuits/cookies) and not dehydrated, freeze-dried, canned or acidified; home-processed products may never claim 'human grade'.
  • Breed/size-specific labels ('for Medium Dogs' vs 'for Large Dogs') are separate $100 registrations.
  • Drug claims, USDA references, unsubstantiated organic/non-GMO claims, and product names not matching ingredients (e.g. 'Lamb' when ingredient is lamb meal) cause denial.
  • Registration applies at farmers' markets and online sales alike; approved registration is mailed back.
  • Mid-year additions use the supplement form with label and payment.
