Forms

Good to know

  • Registration year on the forms page says 'Jan 1 - Dec 31' but the feed page and the form say the registration expires January 31 and the licensing period is Dec 1 – Jan 31; treat Jan 31 as the expiry.
  • Tonnage report form is mailed, custom per registrant, early January; an address change must be reported before the mail-out or the report is lost.
  • Pet-food labeling guide requires a 'Calorie Content' heading (kcal/kg and kcal per treat/cup/can) and metric net weight (e.g. 4 oz (113.4g)); claims to avoid: USDA, vet approved, safe product, no hormones, 100% chemical free.
  • Feed Registrant Search page: 'Updated August 24, 2026'; CSV Last-Modified header Oct 5, 2026 (HTTP 200, 409 KB) — the URL is the same as the May 2026 one.
