Forms

Good to know

  • No firm license and no tonnage — Utah is a pure per-product state ($60/product/calendar year).
  • Calendar-year cycle regardless of when registered; no pro-ration stated.
  • Label review is a pre-market label approval ('The Department reviews each label and declared ingredients').
  • Paper form fields: Customer No., company, address, Contact Name/Phone/Fax/Email, product list (12 rows), signature/print/title/date — the contact block can carry an agent.
  • UDAF renewals for Feed moved to AIMS (agbusiness.utah.gov) per the Renewals page updated Dec 16, 2025.
