Forms

Good to know

  • Firm license only — no product registration and no per-product fee; the recurring cost is $25 license + $50 minimum inspection fee = $75/yr for a small brand.
  • Tonnage minimum $50 is owed even on zero tons; the license will not be issued without the tonnage report.
  • New products: upload PDF labels during the online application/renewal; DATCP reviews labels but does not 'register' them.
  • Explicit consultant/third-party path: DATCP grants online access to the client business after the consultant calls/emails the Feed Program.
  • Renewals in MyDATCP require a PIN issued by DATCP (call/email to get one); first-time users without a PIN cannot renew online.
  • Wis. Stat. 94.72(6)(a) splits the $0.25/ton into $0.23 feed + $0.02 weights-and-measures; the $50 minimum is $46 + $4.
  • Paper filings go to a Milwaukee lockbox (Box 93598), not to Madison; the older tonnage instructions still show PO Box 93193 — use 93598.
